Annual Meeting Registration Fees
Member and Non-member registration fees include the Annual Meeting proceedings, access to scientific sessions, Poster Presentations, Welcoming Reception, Exhibit Hall, morning and afternoon refreshment breaks during the scientific sessions.

* Each company is permitted 2 complimentary Exhibitors per 9m² (9 square meters) booth. Additional registrants should register at the Member/Non-Member rate. Form must be received by August 15, 2011, for participant’s name to appear in SPS Attendees List. Exhibitors must pay to attend CE courses.

Student Registration
The Student registration fee includes the Annual Meeting proceedings, access to scientific sessions, Poster Presentations, Welcoming Reception, Exhibit Hall, morning and afternoon refreshment breaks during the scientific sessions. A letter of verification from a department chairman MUST accompany student registrations. Any student registration received with no verification letter will be processed at the higher NON-MEMBER fee. Copies of student identification cards are NOT acceptable.

Alternatively, registrants paying by check or those not utilizing the Internet may submit their Registration Form by fax or mail.

Forms will be date-stamped as they arrive. This is your date of registration. Faxes will be accepted by SPS Headquarters only if a credit card number is clearly listed in the appropriate area.
NOTE—To prevent double-billing, if you are registering by fax, DO NOT mail your original registration form. SPS needs only one copy for processing. All mailed and faxed registration forms will be processed on-line by SPS staff.
Cancellation Policy
If you must cancel your registration, all requests must be received in writing to SPS Headquarters no later than August 15, 2011. The registration fee will be refunded less a $25 processing fee.
